How do you break up with someone?

My girlfriend and i have been dating for four months but its starting to get hard with school and work we hardly see eachother, i want to break up with her but its so hard everytime i get the chance to break up i freeze and i cant, whys it so hard? does it mean i still love her even tho we hardly talk or see eachother anymore? but how do i do this?
